Rates & Insurance

The rate varies depending on whether you are using your insurance or are a self-pay. Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

  • Do I have mental health insurance benefits?
  • What is my deductible and has it been met?
  • What is the coverge amount per therapy session (co-pay/co-insurance)?
  • How many outpatient visits am I allowed in a calendar year?
  • Is approval required by my PCP?  

Insurance Plans
I am an in-network provider for most insurance carriers including military. Call the office to see if your plan is accepted.

Cash, check, MC, VISA, Discover, AMEX are all acceptable forms of payment. Payment is due at the time services are rendered unless you are referred by your EAP.

Employee Assistance Program (EAP)
All EAPs will pay for a limited number of visits. Contact your employer to see if they have EAP benefits for employees and if I am a provider for your company's EAP.

Cancellation Policy
A missed appointment is a loss for everyone. If you are unable to keep your appointment please kindly give at least a 24hrs notice. Appointments missed or cancelled with less than a 24hr notice will incur a $65.00 cancellation fee (except in the event of a documented medical emergency). This fee is not billable to your insurance carrier and must be paid prior to your next appointment.
